We also have many activities for the participants leading up to the night of our Prom, be sure to finalize your payment ASAP to be included in the Mask Making Workshop, Florist Activity, and other Pre-Prom events for our member families!
Please Note: Download, complete, save (using: file - print - save to PDF) and email updated forms to TeenActivities@gmail.com each school year, per guest attendance. Make sure to bring a new copy of each of the forms with you throughout the school year as a reference in case of emergency only. Teen Activities reserves the right to ask for updated forms throughout the year as needed. Handwritten forms will not be accepted. Properly Submitted forms are required for student/family member admittance to each event. You can download the forms under the “IL Teen Activities Membership” tab above.
