Make sure to bring a new copy of each of the forms with you throughout the school year as a reference in case of emergency only. Teen Activities reserves the right to ask for updated forms throughout the year as needed. Handwritten forms will not be accepted. Properly Submitted forms are required for student/family member admittance to each event. You can download the forms under the “IL Teen Activities Membership” tab above.
